2025 Inland Empire Community Health Needs Assessment

The 2025 Inland Empire Community Health Needs Assessment (CHNA) and its transformational tools bring an opportunity for renewal—a time to study our community’s needs and think about new ways to support each other in our health and well-being. Five hospitals and the communities they serve participated in this assessment, including Chino Valley Medical Center, Montclair Hospital Medical Center, Redlands Community Hospital, San Antonio Regional Hospital, and San Gorgonio Memorial Hospital. The priorities in this report reflect the challenges we face together and are part of the ongoing work to ensure every community member has a chance to thrive. They consider not only the urgent health issues of mental and behavioral health, cardiovascular and diabetes disease, and maternal and infant health, but also the conditions that shape our well-being—basic needs for health and safety and humane housing. These are the foundations of a healthier future for us all.
