Who We Are
Communities Lifting Communities is a 501(c)(3) non-profit affiliate of the Hospital Association of Southern California, working alongside HASC hospital members and multi-stakeholders in co-creating and co-designing strategies to improve community health and reduce health inequities.

Pillars
Foster Collaboration
We foster collaboration between hospitals and health systems, public health agencies, Medi-Cal Managed Care Plans, community clinics, community-based organizations and members and patients because we can do more collectively than individually.
Cultural Competency & Relationships
We improve intercultural competency by understanding culturally appropriate care for patients and families and the intersections of race, gender, identity and language.
Center Patient & Community Voices
We center patient and community voices through active listening about needs, building bridges and relationships to address the social drivers of health, and bringing local resources to patients and families.
Provide Quality Improvement
We facilitate quality improvement by disaggregating patient outcomes data by race, ethnicity and language and implement QI strategies at the clinical, institutional and community levels.
Build Modes for Reflection
We build modes for reflection and assessment to ensure we are achieving the health improvement outcomes we want and to redesign the strategies as needed.
