Cal AIM Academy

CalAIM Academy Overview

The CalAIM Academy for Hospitals and Health Systems was held from February – April 2025.  A partnership of HC2 Strategies, Communities Lifting Communities, and the HASC Education team – and funded by the California Heath Care Foundation – this innovative, six-part web series prepared a cadre of hospital and health system leaders to seize the unique opportunities presented by the state’s CalAIM program.

Each of the Academy’s 90-minute sessions features guest speakers including hospital and health system leaders from across California.

We are in the exploration and design phase of continued/deeper supports for hospitals and health systems on their CalAIM journey, all of which would be free to join. Please fill out the interest form to indicate your interest in these opportunities and to receive more information in the coming weeks and months. 

Session 1: Introduction & CalAIM Overview

By the end of the session, participants were able to…

  • Describe the CalAIM Academy program objectives, curriculum arc, and facilitators.
  • Summarize the history, nuts and bolts, and vision of the CalAIM policy in the context of planned and political developments in the field.
  • Relate the ways that hospitals can engage and benefit from CalAIM through vertical and horizontal integration to their current and future strategies.
  • Identify upcoming funding opportunities and associated supports for PATH CITED Round 4.

Session 2: Hospital's Role in CalAIM Leadership

By the end of the session, participants were able to…

  • Define the hospital’s role and leadership in community-wide delivery systems.
  • Strategize how to gain buy-in and will from others around CalAIM.
  • Illustrate a coordinated, multi-departmental, systems approach to CalAIM implementation within the hospital.
  • Discover how ECM and CS providers can be integrated into the hospital to accelerate connection services.

Session 3: Building CalAIM Infrastructure

By the end of the session, participants were able to…

  • Illustrate a coordinated, multi-departmental, systems approach to CalAIM implementation within the hospital.
  • Articulate re-designed, coordinated workflows for population identification, delivery of care, and care coordination models in the context of planned and political developments in the field.
  • Relate CalAIM’s alignment with solving health system pain points such as throughput, workforce, behavioral health, and homelessness crises.
  • Recognize the ways to build CalAIM infrastructure with sustainability at the forefront.

Session 4: Coordination in Community-Based Health Care

By the end of the session, participants were able to…

  • Describe the roles of Managed Care Plans, community-based providers, and Counties as key partners in a hospital’s work in CalAIM.
  • Bridge the language and data used by community-based organizations, Managed Care, and health care organizations for improved collaboration.
  • Identify cross-sector collaboration and communication structures that accelerate community change and population impact. 
  • Understand closed-loop referral systems and how to redesign and coordinate workflows across organizations.

Session 5: Payment Models for the Future

By the end of the session, participants were able to…

  • Interpret ways to make CalAIM financially strategic and sustainable in the context of planned and political developments in the field.
  • Understand new revenue streams and incentives presented by CalAIM and how they relate to other funding flowing into a community.
  • Connect blending and braiding funding to developing population health/value-based payment infrastructure for underserved populations.
  • strategize how to make the value case for CalAIM to their CFOs and leaders.

Session 6: Bringing it All Together

By the end of the session, participants were able to…

  • Envision how CalAIM can be optimized to advance equity and population health outcomes for complex patients.
  • Reframe the development of delivery systems for CalAIM to be scaled to benefit other populations.
  • Distill how leadership, incentives, workforce, and workflows can align to support investments in CalAIM.
  • Connect the learnings from the CalAIM Academy into actionable steps for advancing their work.
